Transfer Credentials

An application for transfer is considered complete ONLY IF:
A. all financial obligations were settled. To pay balance through discipulus, click MENU - PAYMENTS - PAY TUITION. Chat us at www.facebook.com/amascl for the amount of your balance.

B. all required admission credentials were submitted

If admitted as GRADE 11: Original Grade 10 Form 138 (Report Card), PSA Birth Certificate and original JHS Form 137
If admitted as GRADE 12 TRANSFEREE: Original Grade 11 Form 138 (Report Card), PSA Birth Certificate, SHS F137 and Certified True Copy of JHS F137
If admitted as COLLEGE FRESHMAN: Original Grade 12 Form 138 (Report Card), PSA Birth Certificate, Original SHS F137 and formal 2x2 picture
If admitted as COLLEGE TRANSFEREE: PSA Birth Certificate, Original Transcript of Records from previous school MARKED "COPY FOR AMA" and formal 2x2 picture

C. payment for the following documents were done

Credentials to be requested by transferring-out Senior High School Students

Certificate of Good Moral Character. In discipulus, click MENU - PAYMENTS - PAY OTHER FEES. Choose CGMC
Honorable Dismissal. In discipulus, click MENU - PAYMENTS PAY OTHER FEES. Choose Honorable Dismissal
Form 137 (SF10) (This document will be sent by AMA to the school where he will transfer upon the receipt of the request letter.). In discipulus, click MENU - PAYMENTS PAY OTHER FEES. Choose F137
Submit 2 pieces documentary stamps to the guard-on-duty.

Credentials to be requested by transferring-out College Students
